Participant Waiver and Release of Liability

I, [ ], hereby acknowledge and agree to the following terms and conditions in consideration for being permitted to participate in classes and activities at the Dasha Collective:

1. Assumption of Risk: I understand that participating in yoga, tiny tales, and other activities at the Dasha Collective involves certain risks, including but not limited to physical injury. I voluntarily assume all risks associated with my participation.

2. Health and Fitness: I certify that I am physically fit and able to participate in the classes offered. I will inform the instructor of any medical conditions or limitations that may affect my ability to safely participate.

3. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Dasha Collective, its instructors, employees, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during participation in classes or activities.

4. Photo and Video Release: I grant permission to the Dasha Collective to use photographs, videos, or other media of me taken during classes or events for promotional purposes.

5. Compliance with Rules: I agree to comply with all rules and guidelines set forth by the Dasha Collective and its instructors during classes and activities.

6. Emergency Medical Treatment: In the event of an emergency, I authorize the Dasha Collective and its representatives to obtain medical treatment for me if necessary. I agree to be responsible for any medical expenses incurred.

7. Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of Florida/USA.
